This position that is primarily responsible for working with patients and staff in coordinating all aspects of the patient referral process and for adhering to established timelines and departmental procedures.
Essential Job
Responsibilities:
Reviews, tracks, and follows up on Lab/DI/Referral tracking to meet patient needs according to HCSET policies Reviews, tracks, and follows up on Lab/DI/Referrals per policies and procedures and must be able to meet the mandatory guidelines and requirements Contact the referred to provider and determine whether the appointment was made and kept May call to determine barriers to making appointments and possible solutions Documentation and tracking of follow-up on referrals and other tests to be completed in patient's EMR Inform the patient of further recommendations from the provider or from the referred to provider if he/she is not already aware.
Identify issues and inform referral sources of any updates Be an advocate and resource to the patients as you are tracking referrals Determines urgency of cases as tracking them and reporting to the appropriate person.
Communicate with nurses/medical assistants, and providers as needed.
Provides the highest level of customer service to patients, visitors, clinic staff and fellow employees.
Adheres to customer service policies and standards as set forth by management.
